Yes, I want to participate in the mission work trip to Tijuana, Mexico. I realize that
completing this form is an indication of my commitment to the project and I am
including a $150 non-refundable deposit, which will be applied to the total cost.
I understand that the cost will be $800 per person and that the group is committed to
raise an additional $6,000 for Esperanza International’s work in the Tijuana
community (Program Fees). I am willing to do my own fundraising and to help the
total group raise the necessary funds to make the trip a success. I agree to sign and
follow the behavior covenant written for the Esperanza trip. (This trip is sponsored
by the Kansas-Oklahoma Conference of the United Church of Christ.)
PLEASE NOTE: COMPLETE PAYMENT is due by May 1, 2009. Airfare is NOT
refundable after that date. ALSO NOTE: All participants are required to have a
current U.S. passport for this trip!! (No exceptions!)
Please complete the registration form attahced here (PRINT CAREFULLY) and send it along with your $150 deposit by JANUARY 15, 2009 to the Conference Office as noted.
Be sure to make the check out to “K-O Conference” and note “Esperanza
2009” and the name of the participant in the memo area of the check.
